In what product/application are you experiencing an issue?

What were you attempting to do when the issue occurred?

  • login
  • running reports: provide report name & desired info
  • adding users in the system and or setting up privileges: provide first and last name(s)
  • uploading CBLs, course(s): provide name of CBL(s), course title(s)

Is this happening to only you, you and 1 other(s), all?

  1. If only you and 1 other(s):
    • What is the difference between those that do work & those that do not work?
    • Does it have anything to do with the difference in user privileges’ within
    • application/computer/network?
  2. If all:
    • Did the function/task work properly before or has this always been an issue?
    • If function/task worked properly before, what happened to change it?
